Special Orders

Below is a copy of our Special Order agreement for your
inspection.  If you are still interested in placing an order after
wading through all the mumbo-jumbo, please email  (email
button below) and we will send you the neccessary paperwork.

This agreement is entered into on _______________,2003, and is between _____________________
(*Purchaser*) and Sharon May (DBA Killbuck Creek Quilts).
The Purchaser agrees to buy ____________ from Killbuck Creek Quilts for the purchase price of
$_________ US.  Twenty-five percent (25%) of the purchase price is due at the time of the signing of
this agreement with the remainder due at the time of the delivery of the merchandise as described
below.  Expected date of delivery is on or before _________________.  If purchaser chooses not to
accept completed merchandise, the twenty-five percent down payment is forfeited (retained by Killbuck
Creek Quilts) and the quilt remains the property of Killbuck Creek Quilts.

DESCRIPTION OF MERCHANDISE:

The quilt will be machine quilted, and of the _______________________ pattern which consists of
_______________________________________________ with the following exceptions:
_______________________________________________________.  [NOTE:  If different pattern selected
by Purchaser, specify here and initial by both parties: ___________________________.
Quilt dimension will be approximately_____________________.  The main colors of the quilt will be
______________________ (with________________ as accent color(s).  The quilt will have a
_________________ backing.  A label will be sewn on the back in one corner.  Special label text will be
specified by the purchaser and will be provided to Killbuck Creek Quilts no later than
________________(two (2) weeks before delivery).  All fabrics used will be 100% cotton.  Batting will be
of Purchaser's choice________________________________.  Quilt is guaranteed to be a unique artwork.
 Workmanship is guaranteed except for normal wear and tear expected of bedding materials.
Purchaser understands and agrees that Killbuck Creek Quilts is providing our time and expertise, but
do not guarantee any particular outcome or result except as noted above. Killbuck Creek Quilts will
work with you to try to cooperatively resolve any disputes that may arise between us.  However, if for
any reason there is litigation between us, neither of us will be liable to the other for punitive,
exemplary, special, incidental, or consequential damages.  Similarly, Killbuck Creek Quilts will not be
liable for any amount in excess of the fees actually paid to Killbuck Creek Quilts under this
Agreement.

ACCEPTANCE

This Agreement, including its attachments, sets forth the full and complete agreement of the parties,
and can be modified only by a written document signed by both parties.  This Agreement shall be
interpreted and applied in accordance with the substantive law of the State of Indiana.  To accept the
Agreement, Purchaser must sign both copies of this agreement in the space provided.   One copy will
be retained by Killbuck Creek Quilts with the second copy provided to the Purchaser.
